Requests for support letter/medical reports/forms

Please do not request any paperwork during medical consultations. If you would like a letter of support or require us to complete any form, please complete the attached form and hand in or email to the practice with your attached paperwork. We may not have the capacity or ability to complete all requests but will let you know if we are unable to complete your request. Our email address is ggc.gp46521clinical@nhs.scot. All paperwork can take up to 28 days for us to complete, we will contact you when your report or letter is ready for collection and notify you if there is a charge.
