Register with the surgery

If you would like to register with the practice please fill in the registration form and the new patient form and email back to us on ggc.gp46521clinical@nhs.scot with a copy of your ID and proof of address if possible. If you are registering a child please make sure you fill out the under 5 new patient form and the registration form also. Registration process takes 72 hours if you need medication please get from your previous GP before registering with a new practice. We accept registrations for the G31, G32, G33 and G34 postcode.
